In accordance with Public Health England recommendations, all antimicrobial prescriptions are required to have a clinical indication and treatment duration documented on the prescription and in the case notes. 

How to document on EPMA

1.      When prescribing an antimicrobial, create an antimicrobial therapy note.

2.      Indication and review dates can be documented within the antimicrobial therapy note.

3.      Automatic stop dates can be added to antimicrobial prescriptions at the point of prescribing.

How to document on a paper drug chart

1.      Document the indication for therapy in the ‘additional instructions/indication’ box


2.      Document the stop date or review date in the ‘stop date’ box​
